You may create your Family Portal account and add basic information at anytime. In order to submit your registration, you will need to enter or upload the following required information. Partial registrations will be saved for completion at a later date; however, your child will not be enrolled until registration is complete and payment has been made or payment plan is set up.

• Primary Care Physician name and phone number
• Immunization Records and dates
• Date of most recent physical exam
• Medical History
• Health Insurance Information
• 3 emergency contacts who are not primary guardians
• Allergy Information (if applicable) 
• Medication Information (if applicable)
• Individual Healthcare Plan from school or Dr for upload (if applicable) 
• Doctor's Orders for Medication for upload (if applicable) 
• IEP, 504, etc. for upload (if applicable)
• Custody Information (if applicable)

Returning participants: information will be auto-filled where applicable. Please review/update for accuracy.
